With our app, you can easily add custom fields to your cart page, and optionally make them required. Customers won't be able to visit the checkout area unless they fill out the information you need. The captured information is saved with Shopify and available to you in the order page. It can also be exported, displayed in emails and order printouts etc. The app supports the newest theme format known as Online Store 2.0. All charges are billed in USD. Recurring and usage-based charges are billed every 30 days. Does what it says, does it well and inexpensively, to boot!
